Distribution and Frequency of Leukocyte Counts Based on Pneumonia Severity Index (PSI) Score Among Inpatients with Pneumonia at dr. Suhatman Mars Regional General Hospital, Dumai

Abstract

This study evaluates the correlation between absolute leukocyte counts and pneumonia severity, measured by the Pneumonia Severity Index (PSI), among inpatients at dr. Suhatman Mars General Hospital, Dumai. An observational analytic cross-sectional design was employed, utilizing purposive sampling to select 91 eligible hospitalized pneumonia patients. Data analysis utilized the Spearman Rank correlation test. Demographically, the cohort was dominated by geriatric patients (65.9%). The PSI severity distribution revealed 46.2% in Class III and 33.0% in Class IV. Although reactive leukocytosis was highly prevalent (54.9%), statistical analysis demonstrated a remarkably weak positive correlation between leukocyte counts and PSI severity classes (p=0.006, r=0.289). Consequently, absolute leukocyte counts cannot reliably predict the holistic clinical severity of pneumonia, particularly in elderly populations experiencing immunosenescence. Healthcare facilities must prioritize comprehensive prognostic scoring instruments like the PSI over single hematological parameters for accurate clinical triage and vital therapeutic decision making in medical emergency department settings worldwide today.
